Non-members: One-time standard fee plus the cost of labs and medications dispensed. -
Members: Significantly discounted labs (often less than average co-pays) and discounted medications.

Accepting Patient 18-64 YEARS OLD -
Simple Monthly Membership Per Participant: $195 -
One-time enrollment free equivalent to one month of membership will be waived with 3 month prepayment.

Additional fee at near-cost for labs drawn in the office.
*Reflects discounted pricing when paid with ACH account. Other forms of payment are accepted but enrollment and monthly fees are 4% higher.
**If you decide to cancel, kindly let us know in writing with a 30 day notice. If you cancel then decide to rejoin, you will be asked to pay the enrollment fee again.
***There will be a charge of twice the enrollment fee for members that leave and return within 6 months.

Management of chronic diseases such as diabetes, and hypertension. -
Coordination of care with specialists. -
Convenient e-consulting services through RubiconMD. -
Depression and anxiety management. -
In-office testing — EKG, spirometry, pulse oximetry, urine dipstick, rapid strep, pregnancy test, wet mount, KOH prep, anoscopy, hearing screen, vision screen. -
In-office procedures — suture removal, IUD removal, ear wax removal, abscess drainage, management of nose bleeds.

Home visits ($99 within a 10 mile radius). -
Dermatologic procedures including shave biopsy ($30) punch biopsy ($30), skin tag removal (first 3 free then $2 per tag), cryotherapy (first 3 free then $2 per lesion), cyst removal ($50), hypertrophic scar (Keloid) injection($30). -
Urgent care procedures including wound repair (with stitches or glue, $30), splinting ($40).

Joint injections and aspirations($30). -
Ganglion cyst aspiration($20). -
In-grown toenail removal($40).
