Concierge medicine is personalized, high-end healthcare with higher fees and more services, such as house calls and 24/7 access to the doctor. DPC is affordable, accessible healthcare with lower fees and no insurance acceptance. DPC doctors also typically see fewer patients per day, which allows them to spend more time with each patient. Concierge medicine bills insurance plus a retainer fee, while direct primary care (DPC) never bills insurance.

Primary care-related visits (including acute sick visits) are exclusively available only to our members.​

If your membership isn’t meeting your needs, you can cancel at any time with a 30-day notice. A re-enrollment fee equal to the initial registration fee will apply for former members request to rejoin.

The monthly fee covers all our doctor services including unlimited visits, telehealth and email communication. There are certain procedures (ex. mole biopsy) where the procedure is free, however patients are responsible for any off-site costs (example lab or pathology fees). Patients can choose to use their insurance or pay the low cash fee negotiated by your doctor -which sometimes is cheaper than the cost with insurance!

The one-time $150 registration fee covers the time Dr. Altafi takes preparing for your appointment by obtaining and reviewing your available old records, building your chart and assessing your health. We are currently waiving our enrollment fee for patients established with us > 3 months.

No, we do not bill insurance for our services, although many of our patients do have insurance, they are welcome to use for outside tests, specialist visits etc. We do encourage wrap-around insurance, available at low rates, with slightly higher deductibles. We are unable to provide the highest quality service while operating within the restrictive bureaucracy of the insurance system. You still need insurance for emergencies, specialists, and testing.

One of direct primary care’s primary values to the healthcare system is that we do not accept insurance. With regards to Medicare, we are more than happy to see these patients. This is only under the agreement that neither the physician nor the patient will bill Medicare for the services provided. Medi-Cal/ Medicaid considers us a non-participating physician and we are not in their network, so we are unable to provide services at this time.

Yes! Patients may use HSA/ FSA funds to cover cost of their membership. Always consult with your individual plan for details.

For complex medical issues or conditions that have not responded to the usual treatments, referrals will be placed to the appropriate specialist(s). The practice coordinates care with the specialist and provides them with relevant medical information. Consultation notes are reviewed when available and follow-up care is set up as needed. Visits to specialists, procedures, surgeries, and ER visits are outside the scope of this membership and are subject to the terms of your health insurance plan.

Your membership is activated at the time of enrollment; after this time you have full access to the doctor. The registration fee and prorated month will be paid at the time of enrollment. Ongoing monthly billing occurs on the first of each month.

Dr Altafi works in the spirit of “do no harm” and for this reason we do not routinely prescribe potentially harmful controlled substances such as chronic benzodiapenes (xanax, ativan, klonopin) for anxiety. We also avoid treating chronic pain with opioids. For new patients already established on these medications, we will work with patients to explore safer alternatives and wean off these medications if possible or refer to the appropriate specialist as needed.

Complete DPC strives to always maintain a high level of patient experience. To do this, there is a limit to the practice size (just a fraction of a normal patient panel size of a traditional practice) so that scheduling of urgent issues can be done the same day or next day.
